A system and method that are used for creating a longitudinal medical record for an injured person. The system includes in at least one embodiment, a , plurality of mobile computing devices (104). The mobile computing devices having an interface for receiving information from a first responder or a medic. The mobile computing devices are further connected through a computer network (102) to at least one database (106) at a medical facility such as a hospital.
