Abstract From the Canadian patent

A form made of paper or other suitable material wherein the form has an information carrying section in, a first portion thereof, for recording the information of one or more items detachably retainable by said form and an, attachment means in a second portion thereof. The attachment means releasably retains one or more of the items. Further the attachment means has one or more retention flaps formed in the second portion and having means therein to receive one or more of the items and to frictionally retain same by friction force. - 1 -
